We are open on Saturday mornings alternating between Shaw Heath & Bramhall. Contact reception during the week to check we are open before making a journey if you are collecting a prescription etc

The surgeries are both open from 8.00am until 6.30pm. After 6pm the telephones are transferred to voicemail where you can find details of our out of hours service

 

Online 'Pre-Registration' With The Practice

** Please note: until ALL forms are signed and brought in to the Practice and ID is validated, you are not a patient and you cannot book an appointment **

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  NByou are also requried to provide proof of address in the form of a utility bill and either a passport or other form of ID which contains a photograph.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Fill it out and bring it in with you, together with the other necessary ID documents, when you attend to register at the practice.  Registration must be done prior to booking an appointment.

Named GP: As from June 2015 we are obliged to inform patients that they have a “named GP”. This has no bearing at all on which GP you currently see and / or wish to see in the future.
